Below are some of the forms available from the Electoral Office. Forms must be printed in A4 format and should be originals, not photocopies. Once completed, the form should be sent to your local Area Electoral Office.

If you do not have a printer you can telephone us on 0800 4320 712 or email info@eoni.org.uk with your name and address details, and we will send you a form.

Form to register to vote in Northern Ireland or change your details.

Please read the accompanying guidance notes before completing the form. You may be asked to provide evidence in support of your application. For further information on the type of evidence required, visit the Frequently Asked Questions - Registration page.

To obtain the registration form in a range of different languages please visit the Translations page or contact us and we will send one out to you. The form is also available in Braille, audio tape, daisy disk or large print upon request.

Form to register as an Overseas Elector.

Form to register as a Service Voter (or spouse of a Service Voter).

Form for European electors to register to vote at European elections in Northern Ireland.

This form is to be completed by European electors (not British or Irish) who wish to vote at European elections in Northern Ireland. It must only be completed after the elector has been added to the electoral register through completion of a standard registration form (above).
