Below are some of the forms available from the Electoral Office. Forms must be printed in A4 format and should be originals, not photocopies. Once completed, the form should be sent to your local Area Electoral Office.

If you do not have a printer please contact us and we will send you a form.

Form to register to vote in Northern Ireland or update your details. Also use this form to apply for an Electoral Identity Card. You may be required to submit documentary evidence to support your application - please read the guidance notes attached to the form.

This form may be completed online but the law requires that it be signed personally. Please print off the form to sign or complete the form manually.

To obtain this form in a range of different languages visit the Translations page or contact us and we will send one out to you. The form is also available in Braille, audio CD or large print upon request.

Use this form if you are applying for an Electoral Identity Card by post and do not have any photographic identity to send in with your application. For further information visit the Electoral Identity Card page.

Form to register as an Overseas Elector.

Form to register as a Service Voter (or spouse).

Form to register as a Crown Servant or British Council employee (or spouse) working abroad.

Form for European electors to register to vote at European elections in Northern Ireland.

This form is to be completed by European electors (not British or Irish) who wish to vote at European elections in Northern Ireland. It must only be completed after the elector has been added to the electoral register through completion of a standard registration form (above).
